You will find a link to the Catawba Ridge Absence Reporting Google form below.  In order to access and complete this form, you will need to have a Gmail (Google) account.  The form allows you to upload photos and scans of medical notes/bereavement documentation/etc.  This form will record your email address and send you a confirmation.

If you don't have a Gmail account, you can create one or you can continue to submit parent notes and documentation to me via email (burrisjl@fortmillschools.org).   Please provide the student(s) name(s), the date(s) that your student(s) will be absent from school, and the reason for the absence.  We do not accept call in absence reports.
